Referral Process

Everyone attending the Unique Contact and Community Services has to complete a Referral and Contact Agreement form which needs to be returned to the centre prior to the start of contact sessions.

  • We will then invite you and your child to visit the centre in advance of any contact to meet and get to know our staff, familiarise yourself with our facilities and ask any questions.
  • Referral forms can be downloaded below or we can email or post them to you.
